Tri-County Diabetes Alliance — Peninsula Regional Medical Center, Atlantic General Hospital, and McCready Health Foundation

Using Community Health Assessment data, Peninsula Regional Medical Center, Atlantic General Hospital, and McCready Health Foundation partnered with the local county health departments to provide patient education and screenings for residents at risk of developing or who have been diagnosed with pre-diabetes and diabetes. Education ranges from risk factors and paper screenings to self-management and nutrition for those already diagnosed.

Monthly partner meetings include the three area health departments, the three area hospitals, the Eastern Shore Chapter of the American Diabetes Association and other local providers and organizations. Screening and follow-up are provided to at-risk populations. The group provides education at local health fairs and employer worksites.
